NOTICE OF HIPAA COMPLIANT PRACTICES

THIS NOTICE DESCRIBES HOW YOUR HEALTH INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS IT. PLEASE REVIEW IT CAREFULLY.

Legacy Family Chiropractic is committed to protecting the privacy of your medical information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). This Notice of Privacy Practices explains how your Protected Health Information (PHI) may be used and disclosed, your rights regarding that information, and our legal responsibilities.

This may include sharing relevant information with other healthcare providers involved in your care, such as primary care physicians, obstetric providers, pediatricians, physical therapists, imaging centers, or laboratories, when appropriate.

 

Payment

We may use and disclose your PHI to bill and collect payment for services rendered. This may include communication with insurance carriers, billing services, or other third-party payers. If you are responsible for out-of-pocket expenses, we may also contact you regarding balances or payment arrangements.

Your information may be used for practice operations such as quality improvement, staff training, compliance activities, audits, credentialing, and business management. These activities help ensure safe, effective, and compliant care.

 

Appointment Reminders and Office Communications

We may contact you by phone, email, text message, or mail regarding appointment reminders, care-related follow-ups, or office updates. You may request alternative or confidential communication methods in writing, and reasonable requests will be accommodated.

 

Public Health, Legal, and Regulatory Requirements

We may be required to disclose PHI as permitted or required by law, including for:

Workers’ Compensation and Workplace Injuries

Research and Other Uses

Legacy Family Chiropractic does not sell your health information and does not use PHI for marketing without written authorization. Participation in research or non-routine disclosures will require your explicit written consent.

You may inspect or request copies of your medical records and billing information. Requests must be made in writing, and reasonable fees may apply as permitted by law.

If you believe information in your record is incorrect or incomplete, you may request an amendment in writing. We may deny the request if the record is accurate and complete.

 

Confidential Communications

You may request that we communicate with you in a specific way or at a specific location for privacy reasons. Reasonable requests will be honored.

 

Accounting of Disclosures

You may request a written list of certain disclosures of your PHI made within the previous six years, excluding disclosures related to treatment, payment, or healthcare operations.

 

Restrictions

You may request limitations on how your PHI is used or disclosed. While we are not required to agree to all requests, we will consider them carefully.

You have the right to receive a paper or electronic copy of this Notice of Privacy Practices at any time.

 

Privacy and Security Safeguards

We maintain administrative, technical, and physical safeguards to protect your PHI from unauthorized access, use, or disclosure. Our staff receives training on HIPAA privacy and security requirements.

If a breach involving unsecured PHI occurs, you will be notified as required by law.

 

Changes to This Notice

We reserve the right to revise this Notice of Privacy Practices. Any changes will apply to all PHI we maintain and will be effective on the revision date. The current notice will be available:

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
